Update plugin name

The plugin was previously named "Haft Lang - Theme" in h.json. It is now moved to t.json to take the name "Theme - Haft Lang" to follow the general naming convention of themes: "Theme - <theme_name>"

Will be changing plugin listing name to "Theme - Haft Lang" to follow the general theme naming convention.
